HOW TO REGISTER
Patients coming to the surgery to register will be asked to bring their NHS number and to confirm their current address and proof of ID. Provided the patient is living within the surgery catchment area, they will be asked to fill in an application form with their personal details.
To speed up the process patients can download the required registration documents from below, and bring them completed to the surgery.
To complete the registration you are required to have an appointment with our nurse for a routine health check (age 15 and above). This examination is very brief but gives us vital information for our records. Patients are asked to bring a fresh urine sample to this appointment. If you are on several medications an appointment with the doctor will also be required.
*Please note that for all patients registered at Wallasey Medical Centre the Named Accountable GP is Dr Kitur Patel.

Alcohol Questionnaire (Over 16 years only)
